Description

What is Afinitor and when is it used?

Prescriptened by your doctor.

Afinitor contains the active ingredient everolimus and is used to treat

  • postmenopausal women with advanced, hormone receptor positive, HER2 negative breast cancer in combination with exemestane after failure of treatment with letrozole or anastrozole
  • advanced neuroendocrine tumors that originate in the pancreas, gastrointestinal tract or lungs
  • Advanced stage renal cell carcinoma.

Neuroendocrine tumors are rare tumors that can occur in different parts of the body.

The active ingredient in Afinitor stops the cancer's cells from multiplying and cuts off its blood supply. This slows the growth and spread of the carcinomas.

When should Afinitor not be used?

You should not take Afinitor if you are hypersensitive or allergic to everolimus, to medicines with a similar active ingredient (sirolimus, temsirolimus) or to any other component of Afinitor, such as milk sugar (lactose).

When should you be careful when taking Afinitor?

Afinitor will only be prescribed to you by a doctor with experience in cancer treatment. Follow all of your doctor’s instructions carefully.

Tell your doctor if you:

  • Have lung or breathing problems: Tell your doctor immediately if you notice new or worsening lung or breathing symptoms, such as cough, chest pain, or shortness of breath. This could be a non-infectious pneumonia (i.e. not caused by pathogens), which could potentially have life-threatening effects. Your doctor will decide whether a dose reduction (or interruption of treatment or discontinuation) of Afinitor is necessary or whether you need an additional medicine,
  • have any infection: Afinitor may increase the chance of getting an infection (such as pneumonia or urinary tract infection). If you notice a high temperature, cold or other signs of infection, tell your doctor straight away as you may need urgent treatment for your infection.
  • Allergic reactions: If you develop symptoms such as swelling of the tongue or throat and/or difficulty breathing during treatment with Afinitor, these may be signs of a serious allergic reaction. In such a case, contact a doctor immediately.
  • Renal disease: Renal failure has been observed in some patients receiving Afinitor. Your doctor will monitor kidney function during your treatment with Afinitor.
  • Vaccination: Ask your doctor first for advice if you should receive a vaccination during treatment with Afinitor.
  • Radiation therapy complications: Severe radiation reactions (such as shortness of breath, nausea, diarrhea, rashes, and pain in the mouth, gums, and throat) have been observed in some patients who received Afinitor with radiation therapy or shortly after radiation therapy. Tell your doctor if you have received radiation therapy before treatment with Afinitor or if you are scheduled to receive radiation therapy in the near future. In addition, radiation reactions at the site of previous radiation therapy (e.g. skin redness or pneumonia) have been reported.
  • Have mouth ulcers (inflammation of the lining of the mouth): Afinitor can cause mouth ulcers and pain. Tell your doctor if you have pain, discomfort or open mouth ulcers. Your doctor will decide whether a dose reduction (or interruption of treatment or discontinuation) of Afinitor is necessary. You may also need treatment with a mouthwash or gel. However, some mouthwashes and gels can make ulcers worse, so do not try anything without first consulting your doctor.
  • have liver problems or have had any disease that may have affected your liver (then a reduction in the dose you receive may be necessary),
  • previously had hepatitis B because this can be reactivated during your treatment with Afinitor (see section “What side effects can Afinitor have?”)
  • have diabetes (too high blood sugar level),
  • have high cholesterol levels and/or the levels of other blood fats are elevated,
  • taking certain other medicines such as:
    • Anti-infective agents (against fungal infections, e.g. posaconazole, itraconazole, voriconazole and fluconazole, and against bacterial infections, e.g. antibiotics such as clarithromycin or erythromycin),
    • Medicines used to treat tuberculosis such as rifampicin or rifabutin,
    • Preparations containing St. John's wort extracts (also known as Hypericum Perforatum ) for the treatment of depression and other conditions,
    • Anti-seizure medications (such as phenytoin, carbamazepine, cannabidiol [CBD] and phenobarbital),
    • Anti-HIV drugs such as ritonavir, amprenavir, fosamprenavir, efavirenz, nevirapine,
    • some medicines for heart disease or high blood pressure (such as verapamil, diltiazem) and angiotensin-converting enzyme inhibitors (ACE inhibitors: medicines used to treat high blood pressure or other cardiovascular problems),
    • Ciclosporin (a medicine used to stop the body from rejecting transplanted organs),
    • aprepitant (a medicine used to stop nausea and vomiting),
    • Midazolam (a medicine used to treat acute seizures or as a sedative before or during surgery or other medical procedures),
    • Products containing cannabidiol (CBD) (CBD hemp, cannabis).
  • are pregnant or believe that you are or may become pregnant (see below),
  • breastfeeding your child (see below),
  • You are planning to have a surgical procedure, have recently undergone one, or are experiencing wound healing complications as a result of such a procedure, as there is an increased risk of wound healing problems when using Afinitor.

You should never take a new medicine while taking Afinitor without checking with the doctor who prescribed Afinitor. Please only take Afinitor after consulting your doctor if you know that you have an intolerance to some sugars.

During treatment with Afinitor, your blood values ​​will be checked regularly. The number of blood cells (white and red blood cells, as well as platelets) is monitored to determine whether Afinitor has an undesirable effect on them. These blood samples are also taken to monitor your kidney and liver function as well as blood sugar and cholesterol levels because Afinitor can also affect these.

Tell your doctor or pharmacist if you:

  • suffer from other illnesses,
  • Have allergies or
  • take other medicines (including those you have bought yourself!) or apply externally.

Because Afinitor can cause undesirable effects such as nausea, vomiting and tiredness, this medicine may affect your ability to react, use tools or machines and drive.

Can Afinitor be taken during pregnancy or while breastfeeding?

Afinitor could harm an unborn or breast-feeding child.

Pregnancy: It is recommended not to use Afinitor during pregnancy. Tell your doctor if you are pregnant or think you may be pregnant. Your doctor will discuss with you the possible risks of taking Afinitor during pregnancy. Women of childbearing potential receiving Afinitor are advised to use highly effective contraception (such as condoms or the pill) during and for 8 weeks after stopping treatment. If you think you have become pregnant or are planning to become pregnant, ask your doctor for advice before continuing to take Afinitor. Male patients who take Afinitor do not have to give up trying to father children.

Breast-feeding: It is recommended not to breast-feed your child during treatment and for 2 weeks after the last dose of Afinitor. Tell your doctor if you are breast-feeding.

How do you use Afinitor?

Please follow your doctor's instructions carefully. He or she will inform you how many Afinitor tablets you should take. Do not take more tablets than your doctor has told you.

The usual daily dose is 10 mg (taken once daily).

A higher or lower dose may be recommended by your doctor based on your individual treatment needs. For example, if you have liver problems, your daily dose could be reduced to 5 mg once a day. If you have certain undesirable effects of Afinitor (e.g. lung or breathing problems, mouth ulcers), your doctor may prescribe a lower dose or tell you not to take Afinitor for a while.

Do not stop taking Afinitor without your doctor's approval.

Swallow whole tablets with a large glass of water. Do not chew or crush the tablets.

Afinitor should be taken orally once daily, either with or without food. It is important to take Afinitor at the same time every day so that there is a consistent amount in the bloodstream.

Do not drink grapefruit juice or eat grapefruit. This could lead to an increase in Afinitor in the blood (possibly to harmful levels).

Afinitor should not be used in children and adolescents.

Please contact your doctor or hospital immediately if you accidentally take too many Afinitor tablets or if someone else accidentally takes your medicine. Show the Afinitor packaging. Medical treatment may be necessary.

If you forget to take Afinitor, skip the dose for that day. Then take the tablet again the following day at the usual time. Do not take a double dose to make up for a missed dose.

Do not change the prescribed dosage on your own. If you think the medicine is too weak or too strong, talk to your doctor or pharmacist.

What side effects can Afinitor have?

The following serious side effects are very common (affecting more than 1 in 10 people) when taking Afinitor:

  • Lung or breathing problems (e.g. pneumonia, embolism, or acute progressive respiratory failure)
  • Infections

Talk to your doctor immediately if you notice any of the undesirable effects mentioned above.

Very common (affects more than 1 in 10 users ):

  • Inflammation of the oral mucosa including mouth ulcers (stomatitis)
  • Infections
  • Lung or breathing problems (e.g. pneumonia, embolism, or acute progressive respiratory failure)
  • Stomach problems such as nausea, vomiting, diarrhea,
  • Loss of appetite, impaired sense of taste
  • Headache
  • Cough
  • Nosebleeds (epistaxis)
  • Shortness of breath (dyspnea)
  • Rash, itching
  • Feeling weak or tired
  • Swelling of the arms, hands, feet, ankles or other parts of the body (edema)
  • weight loss
  • reduced hemoglobin value (anemia)
  • increased blood sugar levels

reduced number of white blood cells, which increases your risk of infections; reduced number of blood cells (thrombocytes), which help blood clot; increased levels of blood sugar (diabetes); increased levels of blood fats; decrease in ions/salts such as phosphate and potassium; Dehydration; Insomnia; High blood pressure; Swelling of part or all of the arm (including the fingers) or part or all of the leg (including the toes), heaviness, restricted movement, discomfort (possible signs of lymphedema); bleeding (e.g. in the intestinal wall); dry mouth; Difficulties swallowing; Stomach ache; indigestion; mouth pain; increased liver values; increased kidney values ​​(increased blood creatinine); dry skin; nail diseases; Acne; Rash and painful palms or soles (hand-foot syndrome); skin redness; joint pain; protein in urine; kidney failure (severely reduced urine output); irregular menstruation; Fever; Inflammation of the mucous membranes; Swelling of the eyelids.

Hepatitis B reactivation has been observed in some patients receiving Afinitor. Tell your doctor if you have symptoms during treatment with Afinitor hepatitis B: the first symptoms may be non-specific, including fever, rash, joint pain and inflammation. Other symptoms may include fatigue, loss of appetite, nausea, jaundice (yellowing of the skin and the white part of the eye) and pain in the upper right abdomen. Light-colored stools or dark urine can also be signs of hepatitis.

Pulmonary embolism has been observed in some patients receiving Afinitor. Pulmonary embolism is a condition that occurs when one or more arteries in your lungs become blocked. Symptoms may include sudden shortness of breath, chest pain, or coughing up blood. Tell your doctor immediately if you experience any of these symptoms while taking Afinitor.

Afinitor could reduce the fertility of men.

If you are affected by side effects, if the side effects become severe or if you notice other side effects not described here, you should inform your doctor or pharmacist.

Storage instructions

Store the medicine in the original packaging, protected from light and moisture and not above 30°C.

The tablets are white to yellowish, elongated and have beveled edges.

Each 2.5 mg tablet contains 2.5 mg of everolimus and is debossed “LCL” on one side and “NVR” on the other side.

Each 5 mg tablet contains 5 mg of everolimus and is debossed with “5” on one side and “NVR” on the other side.
